Supplementary Information Form

The Governors are the Admissions Authority for the school, and you are asked to complete a Supplementary Information Form, so that the Governors can consider your application in terms of the School Admissions Policy.

 You will see that allowances are made for the current Covid19 situation, regarding Mass attendance. The school welcomes applications from families of all faiths, and families of no faith. All are welcome!

The application process for the Reception intake in September 2021 begins on the 1st September 2020 and you must complete your application and submit all documents by 15th January 2021.  You will be notified of the outcome, by the local authority, on 16th April 2021.

Applications for a transfer between schools, or if you are new to the area, can be considered at any time in the year. You must complete an In Year Admissions Transfer request with the local authority, and a Supplementary form.

Please note that if your child is transferring from another local Lewisham school, transfers can only take place at the half term break, unless there is an exceptional reason., and is agreed between both headteachers.  

If you do not attend a church, you do not need to complete the Priest or Minister's part of the form, but you must still complete a Supplementary Form so that Governors can consider your application. 

You will also need to provide:

  • Your child's Baptism certificate
  • Your child's long birth certificate
